FamilyCareRecord Questionnaire · About 15 minutes

What is looking after someone actually like?

These are the same questions I ask when we talk. Answer as many or as few as you like. The boxes for writing are the ones that help me most, and a sentence is plenty. There are no right answers, and I am not selling anything.

Please do not include anyone's name, address, date of birth or NHS number, including your own relative's. "My mum" or "my father-in-law" is all I need.

About your situation
Who do you look after?
How long have you been caring for them?
Roughly how many hours a week does it take?
Who else is involved in their care? Tick all that apply.
How far away do you live from them?
Are you in paid work?
If you work, does your employer know you are a carer?
Keeping track
Where does the information about their care live right now? Tick all that apply.
If a paramedic arrived tonight and asked what medications they take and what they are allergic to, what would happen?
In the last twelve months, have they been admitted to hospital?
What were you given, what was missing, did anything about their medication change, did anyone follow up?
Which paperwork have you had to produce in the last year? Tick all that apply.
Roughly how much have you paid out of your own pocket in the last year for anything to do with their care? Care hours, equipment, taxis, a key safe, a solicitor, apps, anything.
The idea

Here is what I have in mind. Please tell me where it is wrong.

One place that holds everything about your relative's care: Their medications, appointments, who is doing what, the hospital letters, the council paperwork. You do not type it in. You forward the email, or take a photo of the letter or the pill box, and it fills itself in and shows you where it got that from.

Then it hands that information to whoever needs it, in the form they already use: A printed sheet for the fridge with a code a paramedic can scan to see medications, allergies and who to call. The same summary on the phone's lock screen. A link you can text to the GP or the care agency. A one-page summary for the hospital, or the evidence for a carer's assessment, ready for you to check before it goes anywhere. And when someone scans the code, you are told.

The rest of the family sees as much or as little as you decide, and the ones who never open anything get a short summary once a week.

If a paramedic or doctor needed your relative's information in a hurry, which of these would you actually use? Tick all that apply.
Which of these would you have needed in the last three months? Tick all that apply.
What would worry you about something like this? Tick all that apply.
The basic version would be free. A fuller version, with a phone line for your relative and unlimited records, would be £9 a month for the whole family. What goes through your mind?
If your employer offered this as a benefit, how would you feel about them knowing you had used it?
Staying in touch (optional)
Leave it blank to stay anonymous. Your answers are just as useful to me either way.
If you gave an email, which of these would you be open to?

Nothing is saved until you press the button. If you would rather talk than type, book a conversation with me instead.
